It allows our clinical team to review your medical history, current medications, and the details of your condition before you arrive so that your initial evaluation is as thorough and efficient as possible.

What to Bring to Your First Visit
A valid photo ID (driver's license or state-issued ID)
We verify the identity of every patient at check-in. This is a standard requirement for medical offices and ensures that your medical records, prescriptions, and insurance claims are accurately matched to you.
Your insurance card (front and back)
We verify your insurance eligibility and benefits before your first visit begins. Having your card on hand allows our team to confirm your coverage, check for any co-pay or authorization requirements, and ensure there are no billing surprises.
A list of current medications and dosages
Your provider needs to know what you are currently taking so they can avoid drug interactions and make informed decisions about your treatment plan. This includes prescription medications, over-the-counter supplements, and anything you take regularly.
Any relevant imaging (MRI, X-ray, CT scan) on disc or digital copy
If you have had imaging done previously, bringing those results helps our providers assess your condition without ordering duplicate studies. This saves you time, avoids unnecessary radiation exposure, and gives us a baseline to compare against.
Your spouse, POA, or anyone involved in your healthcare decisions
After your evaluation, our team will review your findings and discuss treatment options with you. Having your spouse, power of attorney, or a trusted family member present ensures that everyone involved in your care is informed and can participate in the decision-making process together.
